SETH MEDICAL WAIVER **Only need if player isn’t registered online!

All teams must register online and pay with a credit/debit card.

You should receive a confirmation email once complete. Check your spam as it may end up in there.

Please Email your proof of age in one PDF to soccer@sethernstsoccerslam.org with Team Name/Age Group in the Subject Line. This will make Team Check-in a breeze!

A late fee will be applied for registrations past the due date, entry fee is $35/player plus handling fee. Included in your $35 is a guaranteed 3 games, one round of our Six Shot & two rounds of our Speed Shot Contests, and a Seth Ernst Soccer Slam T-Shirt.

DATES OF PLAY 2027:

SAT. JAN. 30^th               BOYS Ages 11 – 14/15 (can’t be in HS)

 SUN. JAN. 31st               GIRLS Ages 11 – 14/15 (can’t be in HS)

 SUN. JAN. 31st               GENESEO HS MEN & WOMEN

 SAT. FEB. 27th             BOYS Ages 8 – 10 and U2OLD

                              (Geneseo Alumni, residents of District 228 & Seth Coaches)

 SUN. FEB. 28th               BOYS & GIRLS Ages 6/7 and GIRLS Ages 8 – 10

*Friday Evenings are only for local teams to play as needed.

Team Roster Sizes:
All teams can register up to 8 participants.

*Ages 6/7 – 10 PLAY 4 v 4. All other divisions play 3 v 3

IMPORTANT: We follow the IYSA Birth Year requirements for 2027, which is CHANGING, SO STAY TUNED FOR MORE INFORMATION ON GRADE IN SCHOOL AS AGE REQUIREMENT!

**A copy of birth certificate, pic on phone, OR a current player card is required at check-in. OR…email your proof of age in one PDF to soccer@sethernstsoccerslam.org with Team Name/Age Group in the Subject Line.

The medical waiver is ONLINE – just click a box & all regisered players are covered!

If you do not register ONLINE, a waiver must be signed at Team Check-in along with proof of age.

• This Division is restricted to residents of District 228, Seth Coaches, and Geneseo HS Alumni.
• Teams must be CO-ED and play on Saturday, February 28th.
• Each team may register 8 players with 1 of them being a “guest” player (must be out of HS).
